If you choose not to use your social security number,
leave that section blank and a number will be assigned to you. The Social Security Number
is being requested on a voluntary basis and will only be used for the express
purpose of identifying guests. You may elect to have the Loveland Parks
and Recreation Department Golf Division assign you a 9-digit number.

Once your application form is complete, please click the
"submit" button. An automatic response will be generated to let you know that we have received your application form. You will receive your golf identification card with your
personal identification number and will be able to use the Automatic Tee
Time Reservation System within five (5) business days after receipt of
your application form.
